Is this possible? I get my TV via IP into a STB, which only has a HDMI output. Is there a way to get this into my HTPC (server) and stream it to my extenders? Or should I look for option to convert from HDMI to an analog signal (very expensive + quality loss) and use this input?

HDTV signals over HDMI are normally protected by HDCP to prevent exactly this. Even if it was possible, this would give you the raw video with no compression at all which would need absolutely enormous amounts of disk space.
I have also only seen one HDMI 'capture' card and it is not clear if it would work in this case due to HDCP. Note: this card has no Media Center drivers.
There is now a Component video capture card from Hauppauge. Component has no HDCP protection so it would at least work but this card again has no Media Center drivers. Some set-top boxes are/are going to disable component output to prevent this also.

The signal is not HD, but I can't find anyone who has had succes with a HDMI capture card, as this is not recognized as a tuner in MCE. I have talked to the ISP, and I might have an option changing the STB to a model with SCART output. This I should be able to convert to S-VHS+Audio, right?

Not all SCART connectors support S-Video which would be the best option, but if not it will support converting to composite (not to be confused with component) and you should be able to use composite plus stereo left/right in to an analogue capture card on your PC.
