Neither.
It will approve any updates synchronized *after* you enable the rule, regardless of whether the update is needed in your organization or not. Presumably one would only engage an auto-approval rule on updates you reasonably expect to be needed. For example, I have an auto-approval rule on all Defender updates and Exchange IMF updates.
It will not retroactively approve updates that already exist on WSUS when you enable the rule. For those updates, you will still need to manually approve the updates that are needed.
