There are more knowledgeable people than me on this forum, but until they answer, I would say that the WSUS server cleanup wizard can take a VERY long time to run. I seem to remember it took hours the first time I ran it. I usually run it each month now, after the monthly updates are finished, and it can take 30 minutes to an hour. Speed obviously depends on both the server processor(s) and ram.
SQL Server holds on to the ram it has allocated until the system starts running short of physical ram. But if it's continuing to use CPU, it could be rolling back the changes that had been made before you cancelled the operation.
I've never been able to hazard a guess at why it runs so slowly.
