First.. has this machine always been a DC, or did you recently promote it?
Second, sounds like one of three possibilities: [a] The password on the IUSR_machinename account has been changed, and thus all access to IUSR_machinename is being blocked for anonymous users, and/or [b] The IUSR_machinename account is no longer in the correct groups, specifically the Domain Users group, and/or [c] Permissions on the filesystem have changed, thus denying access to non-admin level accounts.
Note: Not sure where you found instructions to "Delete the IUSR_machinename account and restarting IIS to recreate it", but that certainly may have complicated things!
It may well be the only 'fix' is to uninstall WSUS and IIS, and reinstall IIS and WSUS. (You can keep the database and content, so this process will only take about 20-30 minutes.)
