It depends on how complex your environment is. If it's just a single WSUS server using MSDE then you shouldn't have any problems. Your database IS backed up when we upgrade, but I like to use the wsusmigrate utilites as well as wsusutil to also backup the sever. If something goes wrong, you can just install from scratch and import all your settings and metadata back in.
