No Titles Turabian Format Citation

There are no titles in the citation for Turabian (or Chicago) and there
should be, at least for the first footnote. I could suppress the title for
the next footnotes if it showed up at all. I assume it must be a flaw in the
logic of the code somewhere. Any suggestions?

More generally, there's no provision in Word for Chicago footnote
style at all -- it expects you to use author-date (and it then
implements it extremely poorly).
