You can load the template that contains the toolbar as a global template. In the Developer ribbon, click Document Template. Under Global templates and add-ins, click Add, etc.
If the original location is Normal.dot, you'll probably want to copy the toolbar to a dedicated template (using Word 2003) to make it easier to redistribute & deploy.
