"Custom Columns" will only appear if you have created some custom fields for the entity (i.e. Account, Business Contacts..etc) you are querying on.
Custom fields are created by clicking the 'Customize this Form' button on any of the entity forms. Then, you can go back and query the reports for that entity and you will see the 'Custom Columns' appearing.
