From the Main Outlook window, on the menu select, "Help | About Microsoft Outlook | Disabled items". If you see Business Contact Manager in the list, enable it and restart Outlook.
If you still do not see the toolbar, select "File | Data File Management..." and make sure that you see "Business Contact Manager" listed.
