Do you know for sure that the add in is still installed and running? It may have just left behind toolbar and menu customizations. A common problem.
What you can do is try to interact with it. If it seems like it's dead, it probably just never deleted its toolbars. You can right click any toolbar and hit customize then delete any toolbars or menus that might remain.
