How do you open a new document where there are no styles in the stlye sheet,
so you can create your own?

Every new document that you open is based on the "normal" style. You can
create any style you like based on the Normal style and save it with a name
of your own.

You really can't. All documents/templates will contain all of Word's
built-in styles. You can create your own custom styles as desired, though in
many cases it is preferable to use the built-in styles (see
http://www.shaunakelly.com/word/numbering/UseBuiltInHeadingStyles.html).

In Word 2002 and 2003, you can select exactly which styles you want
displayed (using the Custom setting in the Styles and Formatting task pane).
In Word 2003, through formatting protection, you can also limit which styles
can be used.

In Word 2000 and earlier, the Styles list displays "styles in use." By
default, these styles include Normal, Default Paragraph Font, and Headings
1-3. As other styles are used, they are added. Unused styles can be removed
through the Format | Styles dialog. Display "Styles in use," select an
unwanted style, and press Delete. User-defined styles are actually deleted;
built-in styles are not really deleted (they are just reset to their default
Normal.dot formatting), but they are removed from the list displayed in the
Styles dropdown.

Is there a simple way, globally in either a Document or a Custom Template
(Word 2003) , to delete all styles that CAN be deleted, so you can get rid of
all failed customization attempts or accidentally created List Numbering
formats, etc., and start over from scratch as nearly as possible?

Using the organizer (ALT+F8 > Organizer) open the document/template in the
left window (if not already open). Select all the styles and click delete.
All the styles that can be deleted will be. For the remainder you will get
warning prompts.

As noted below, you can also do this through the Styles dialog. In Word
2003, in the Styles & Formatting task pane, choose Show: Custom, which will
open the Format Settings dialog. Click on Styles... and choose "Styles in
use." You will be able to delete all the styles except Normal, Default
Paragraph Font, and any Headings that are displayed (Headings 1-3 by
default). The styles aren't really deleted; they are just reset to their
default (out-of-the-box) Normal.dot settings, and they are no longer
displayed as "in use." Beyond that, you use the Format Settings dialog to
determine which styles you want to be displayed as "Available."
