Restarting lists in WordprocessingML generated via XSLT

Does anyone know definitively how to get numbered/outline lists to restart
in a WordprocessingML document generated via XSLT?

We have tried styles with w:start, lists with w:startOverride, and various
other options blogged around the web. We are trying to do this without
having to create a new listDef for every list in our documents, as some of
our documents can be quite long, and the lists quite extensive.

We have followed the instructions in the Oreilly Office 2003 XML book, and
other people's writeups to the letter with no success.

Any help or advice that can be offered will be much appreciated.
