stop control toolbar from opening every time

I have created a form in Word with radio buttons. Every time I open the
form, the toolbox appears, and I have to keep switching off the design mode.
How can I stop this from happening?

You have inserted ActiveX controls from the Controls toolbox. These controls
are really intended for UserForms, which are invoked with a macro. Such
controls also trigger Word's Security protection when you have Macro
Security set to High.

If what you want is a form that users can fill out online, you need to use a
protected form, with form fields from the Forms toolbar; see
http://word.mvps.org/FAQs/Customization/FillinTheBlanks.htm and especially
the forms tutorials by Dian Chapman that this article links to.
