I created an Accde file but my Custom Groups don't show
in the Navigation Bar. Is this by design?

i've noticed the same custom group problem. i've got around it by
designing custom ribbons for users to use as opposed to having them use
the navbar. you gain more desktop space by doing such, but it is a
rather complicated method to doing it, involving xml and callbacks.
