How can I "bubble up" changes in a Word doc in a cad dwg?

We attach Word documents in ACAD drawings and need to revise the documents
from time to time.
we normally surround any changes to the drawing with semi-circle "bubbles"
to highlight the change. How can I do this in the Word document?

You could draw unfilled ovals around them, but those would be part of the
document. Alternatively, if you have Track Changes turned on and Word 2002
or 2003 with balloons enabled, you get a similar functionality.
