You've got the fields for Levels 1 and 2 reversed in the Level 3 "number format." Clear that whole box for Level 3 (just select everything and press Delete) and start from scratch. When the box is empty, click on the arrow beside "Previous level number" and choose Level 1. This will insert a gray 1. Type a period. Then insert Level 2 and a period. Then change the "Number style" from "(none)" to "1, 2, 3 ..." This should put you right.
