Some printers have problems with custom page sizes. I've never had much luck with mine, so what I do is start with a standard page size. Then I adjust the margins so that the useable area is the custom size I want. The printer thinks it's printing on standard size - it's too "dumb" to figure out what paper is in the manual feed tray...
