Hello Debs
Where is the form located? Dont you have Manifest.xsf ? Or if you have only
xml file Check whether you have design a form option in file menu. This will
create a new design of the form.
Or if you have the form published to Sharepoint, You can select Modify
Settings and columns|General Settings|edit template
