This newsgroup is about VBA - Visual Basic for Applications - which is the programming language for Word 97-2003 (macros). There is a longer FAQ with more questions and links that is regularly posted in the non-VBA Word newsgroups. You may want to check that out as well.

Userforms are custom dialog boxes written using vba. If you want a fill-in-the-blanks form you want what Word calls an online form.
